第一集 算筆記吧

快10年了吧，重溫House.

從開始以為是腦瘤brain tumor，到做MRI時對gadolinium過敏停止呼吸不得不開喉（不知道這麼說對不對），到懷疑是血管炎vasculitis用steroid後惡化到看不見還差點死掉，再到最後證明罪魁禍首其實是每天兩片藥就能搞定的豬肉絛蟲，剛看了心術的我不禁開始想，這要是放在國內醫療劇里，醫院得賠多少錢啊？醫生很可能早就會被罵是庸醫、拿活人做實驗，然後想方設法四處逃命了吧。


House在門診看的那個有哮喘的小男孩的媽媽說不想給這麼小的孩子多用藥，就沒按醫囑，結果孩子不舒服又跑來看醫生。House好無奈。很多醫生都很無奈吧。前幾天在醫院還聽到一個醫生氣呼呼地對病人說，你不按我說的去養，就別來找我看病。病人可能也很委屈，我也不是全沒照做，只是沒全做嘛。

Terms in plain English:

Asthma: "A stimulant triggers cells in one's airways to release substances that inflame the air passages and cause them to contract. Mucus production increases, the cell lining starts to shed. Steroid stop the inflammation."

Infarction: it's what happens when blood flow is obstructed. If it's in the heart it's a heart attack. If it's in the lungs, it's a pulmonary embolism(肺動脈栓塞）. If it's in the brain, it's a stroke.
Myocardial infarction: 心肌梗塞.

關於人生這點事，第一點就是House堅信的那句everybody lies。這輩子，總會因為這樣那樣的原因撒謊，記住，white lies也還是lies。

第二點，關於生死。"Our bodies break down, sometimes when we're 90, sometimes before we're even born. But it always happens and there's never any dignity in it. It's always ugly. ALWAYS. We can live with dignity. We can't die with it."

第三點，醫患矛盾："Patients always want proof. We are not making cars here. We don't give guarantees."

很好奇，到底搶救或者手術中出現意外時，醫生到底是否應該要立刻跟患者家屬溝通、取得同意。家屬不懂醫，這溝通要如何進行？如果解釋完了，家屬好不容易簽字了，人已經救不活了，怎麼辦？如果不用家屬簽字，醫生又有怎樣的權利替患者決定取捨？
